The Health Behavior Change course is designed to help you:

  • Understand the Personal Health Planning process including:
    • Explore intersection of patient self-assessment with medical recommendations
    • Create measurable and meaningful patient goals
    • Follow-up with patient action on goals over time
    • Identify strategies to support patient ongoing success and close a series of health behavior change visits
  • Identify resources to implement Personal Health Planning in practice

The Health Behavior Change Practicum is designed to help you:

  • Apply patient engagement skills, the Personal Health Planning process and use of Personal health Planning resources to patient scenarios for health behavior change
  • Learn from feedback on mentored practice sessions

Are there any text books or other materials required for this course?

The entire training takes place online and by telephone. There are no required textbooks or other materials.

What are the minimum hardware/software requirements to access the online training?

To access and complete the online training modules the minimum system requirements are as follows:

Internet Connection

  • 0.5 Mbps (recommended 1+ Mbps), wired connection or Wi-Fi connection

  • Adobe Acrobat Reader

Windows Operating Systems

  • 2.33GHz or faster x86-compatible processor, or Intel® AtomTM 1.6GHz or faster processor for netbooks
  • Microsoft® Windows® XP (32 bit), Windows Server 2008 (32 bit), Windows Vista® (32 bit), Windows 7 (32 bit and 64 bit), Windows 8 (32 bit and 64 bit), or Windows Server 2012 (64 bit)
  • Internet Explorer 8.0 or later, Mozilla Firefox 17 or later, Google Chrome, or Opera 11
  • 512MB of RAM (1GB of RAM recommended for netbooks); 128MB of graphics memory

Mac OS

  • Intel CoreTM Duo 1.83GHz or faster processor
  • Mac OS X v10.6, v10.7, v10.8, or v10.9

Chrome OS

  • If you are using a Google device such as a Chromebook the device will display the course PDF documents with its internal software. Adobe Acrobat need not be installed on the system.

Before beginning, please make sure you have done the following.

  • Enabled JavaScript
  • Disabled Pop-up Blocker
  • Depending on your organization’s security settings you may need to make the learning center URL a Trusted Site

Is there homework?

No homework is required. As part of the online training modules, you will be provided with additional online resources which you can access as needed. These are supplemental resources that you may find beneficial, but they are not required for course completion.

Are continuing education credits available?

Yes.

The Health Behavior Change course is approved for a maximum of 3.75 AMA PRA Category 1 Credit(s)™, 3.75 ANCC Contact Hours, 3.75 ACPE credit hours, and 0.4 IACET CEUs.

The Health Behavior Change Practicum is approved for a maximum of 6.0 AMA PRA Category 1 Credit(s)™, 6.0 ANCC Contact Hours, 6.0 ACPE credit hours, and 0.6 IACET CEUs.

Is there an examination?

Health Behavior Change: Yes. In order to receive Joint Accreditation continuing education credits an examination at the end of the course is required. In order to receive credit for the course you must pass the examination with 80% of the answers correct. You may take the examination as many times as needed in order to pass.

Health Behavior Change Practicum: No. In order to receive Joint Accreditation continuing education credits participation in the four practice calls is required.

What kind of official documentation will I receive at the completion of the training?

After completing the course you will receive a downloadable electronic certificate showing the Joint Accreditation continuing education credits earned for the course.

What is the cost of the course?

Health Behavior Change: The course registration fee is $ 397.00

Health Behavior Change Practicum: The course registration fee is $ 597.00

What is the course refund policy?

Health Behavior Change: Online programs are non-refundable and non-transferable.

Health Behavior Change Practicum: For programs with a designated start date, transfers are not allowed. Cancellations may be requested using the cancellation request form. If you make this request at least 7 days prior to the start of the program you will receive a refund of your payment minus an administration fee equal to 10% of the registration fee. If you request a cancellation fewer than 7 days before the program start date or do not arrive on your scheduled start date you will forfeit your payment.
